New Student Registration

Welcome to Vassalboro Community School! We are excited to welcome your family to our school community and look forward to partnering with you in your child's education. To complete your student's registration, please fill out the required registration forms listed below and return them to the school office. If it is more convenient, paper copies of all forms are available at the school office. • Release of Records

• New Student Registration form

• New Student Authorization form

• Transportation Information form

• Residency Affidavit (with two supporting documents) 

• Release of Information form

• Student Health Information

• Health Conditions form

• Permission for Administration on Non-Prescription medications

• Home Language Survey

• Technology Permission form

• Title 1 form
